30-Day Notice of Cancelation Form

• This is for swimmers who are quitting swim team or leaving for longer than two months.
• When you registered your swimmer, you electronically signed a Financial Responsibility Contract. In that contract, it states the following:
• At the time of cancellation, you will be charged for any unfulfilled fundraising for any seasons in which your swimmer has participated.
• At the time of cancellation, you will be charged for any incomplete Team Service Credits.

HOLD Request Form

• This will pause billing for TWO months.
• Your swimmer will be "active" on the rosters. This will hold his/her spot in that training group.
• You will continue to receive communications and updates from the team and your child's coach.
• HOLD memberships work as a credit on your account in the amount you would pay for monthly dues for two months. Other charges (meet fees, equipment fees, etc) will still be billed as normal.
